Dunkirk High School Fall Sports Physicals

Any Dunkirk student-athlete who intends on competing in athletics this fall, and has not had a sports physical for the fall season, must report to the Dunkirk High School nurse's office on August 15 at 9 a.m. for pre-screenings and physicals.

All students need to pick up an Athletic Health History form from the nurse's office and have it signed by a parent/guardian prior to arriving on August 15. If the form is not filled out or properly signed - no physical will be given.

If a student does not have physical clearance as of Monday, August 18 (first day of practices for varsity and JAYVEE teams), they will not be able to begin practicing, and the physical will be done at the discretion and expense of the student.
