MAYVILLE - Brian D. Boltz of Mayville died at Saint Vincent Health Center Erie on Sunday, July 1, 2012 after a short illness. Born in Westfield on September 29, 1970, he was the son of Barbara Webster Boltz of Mayville, and Joseph T. Boltz of Webster NY. Besides his parents, Brian is survived by a sister Tara L. Boltz of Mayville, and several uncles, aunts and cousins.
No public visitation will be observed, and flowers are gratefully declined. Memorials may be made to the Portland Volunteer Fire Department. A celebration of life and committal service will be held at the Port-land Evergreen Cemetery on Saturday, July 21, at 11:00 am. Arrangements by the Freay Funeral Home in Mayville.
